I finally got my Nuclear reactors up and running and realized that I misplaced one of the reactors. When I deconstruct it, I end up picking up the Fuel Rods which as expected exposes me to a lot of radiation. The hazmat suit protects me but even after dropping the fuel rods, my radiation level doesn't drop and my filters eventually get consumed. Is this by design that I am permanently irradiated and I have to die?

Probably because you also got nuclear waste, which is also very radioctive. And since it has a blank icon just like a lot of other items, you probably carried that without realize.

Nope, didn't have any waste generated yet. I did find a workaround though. If I fill my inventory, the deconstructed items go into an overflow crate, which is not radioactive, and then I can simply trash the items directly.
